Summer Camp Waiver, Release of Liability & Consent
The following sections outline important information regarding participation in the Summer Camp Program. Please review each item carefully and check each box to indicate your understanding and agreement.
Behaviour Expectations & Dismissal:*
- I understand and agree that my child is expected to behave respectfully, responsibly, and safely at all times while participating in the Summer Camp Program. Disruptive, unsafe, or inappropriate behaviour will not be tolerated. I acknowledge that such behaviour may result in dismissal from the camp at the discretion of camp staff. In the event of dismissal, no refund will be provided, and the parent/guardian will be responsible for arranging immediate pick-up of their child.
Assumption of Risk:*
- I acknowledge that participation in summer camp activities involves inherent risks, including but not limited to physical injury, illness, or other unforeseen events. I voluntarily assume all such risks on behalf of myself and my child and accept full responsibility for any injury, loss, or damage that may occur as a result of participation in the program.
Release of Liability, Indemnification & Hold Harmless:*
- I hereby release, indemnify, and hold harmless the Township of Macdonald, Meredith and Aberdeen Additional, including its Council, employees, camp counsellors, volunteers, agents, representatives, venues, contractors, successors, assigns, and all other parties involved in hosting or delivering the Summer Camp Program, from any and all liability, claims, demands, actions, or causes of action arising out of or related to my child’s participation in the program. This release includes, but is not limited to, claims for injury, illness, damages, losses, or expenses, whether caused by negligence or otherwise, to the fullest extent permitted by law.
Transportation Consent:*
- I understand that supervised transportation between venues is included as part of the camp schedule. I grant permission for my child to be transported by camp-provided bus or vehicle as required for program activities. I hereby release the Township of Macdonald, Meredith and Aberdeen Additional and all related parties from any liability arising from or in connection with such transportation, except where prohibited by law.
Medical Authorization:*
- In the event of a medical emergency, I authorize camp staff to seek and obtain emergency medical treatment for my child, including contacting emergency medical services (EMS) and arranging transportation if necessary. I understand and agree that any costs associated with medical care are the sole responsibility of the parent/guardian. I confirm that all medical conditions, allergies, medications, and other relevant health information have been fully disclosed on this registration form and acknowledge that failure to provide complete information may increase risk to my child.
Privacy & Use of Information:*
- I acknowledge that personal information collected through this registration will be used solely for the administration and operation of the Summer Camp Program and will not be shared with third parties except where required for safety, medical, or legal purposes.
Personal Belongings:*
- I understand that the Township is not responsible for lost, stolen, or damaged personal belongings brought to camp.
Permission for photos*
- I give my permission for my child to be in photographs taken when participating in summer camps to be used at the discretion of the township.
- I DO NOT give my permission for my child to be in photographs taken when participating in summer camps to be used at the discretion of the township.
