2026 Youth soccer Program

Divisions (by birth year): Intro: 2021 and 2022 Division 1: 2019 and 2020 Division II: 2016, 2017 and 2018 Division III: 2013, 2014 and 2015 Division IV: 2008to 2012 (if 19 in 2026 you are eligible) Soccer will be held Tuesday and Thursday nights and games will consist of home and away games with Desbarats and St. Joseph Island. Soccer runs for the month of July.

Behaviour Expectations & Dismissal:*
- I understand that my child is expected to behave respectfully, responsibly, and safely while participating in the program. Disruptive, unsafe, or inappropriate behaviour will not be tolerated. I acknowledge that such behaviour may result in dismissal from the program at the discretion of program staff or volunteers. In the event of dismissal, no refund will be provided, and the parent/guardian will be responsible for arranging immediate pick-up of their child.
Assumption of Risk:*
- I understand that participation in recreational sports involves inherent risks, including but not limited to physical injury or illness. I voluntarily assume these risks on behalf of myself and my child and accept responsibility for any injury, loss, or damage that may occur as a result of participation.
Release of Liability:*
- I hereby release and hold harmless the Township of Macdonald, Meredith and Aberdeen Additional, including its Council, employees, program staff, volunteers, agents, and representatives, from any liability, claims, or demands arising from my child’s participation in the program, to the fullest extent permitted by law.
Medical Authorization:*
- In the event of a medical emergency, I authorize program staff or volunteers to seek emergency medical treatment for my child, including contacting emergency medical services if necessary. I understand that any costs related to medical care are the responsibility of the parent/guardian. I confirm that all medical conditions, allergies, medications, and other relevant health information have been fully disclosed on this registration form.

- I understand that the Township is not responsible for lost, stolen, or damaged personal belongings brought to the program.
